I, the undersigned individual, parent or legal guardian, grant TAR STARS LIMITED d/b/a TARS STARS (“TARS STARS”) permission to use my or my child’s:

  • photograph;

  • video image;

  • first name;

  • testimonial;

  • artwork; and/or

  • likeness

for promotional, educational, marketing, fundraising, social media, website, newsletter, and “Our STARS” purposes.

I understand and agree that:

  1. Participation in media or promotional materials is voluntary.

  2. TARS STARS may use approved materials in:

  • websites;

  • social media;

  • printed materials;

  • presentations;

  • fundraising campaigns;

  • newsletters; and

  • other promotional content.

  1. No compensation will be provided for use of approved materials.

  2. I release and hold harmless TAR STARS LIMITED d/b/a TARS STARS from claims relating to authorized use of such media.

  3. I may revoke this authorization in writing for future use; however, materials already published or distributed prior to revocation may continue to be used.

  4. TARS STARS may edit, crop, or modify submitted media for formatting or presentation purposes.
